Signing off forms

I have an image I want to put on my website of two people I don't know but can track down, at the time I asked them if I could take the picture but didn't think of asking if it would be ok to put it on my site or even sell it on, I've not seen a signing off form before if someone has got a sample of one would you be so kind to e-mail it to me (word-doc) cheers JT :thumbsup:
